Requirements

  • Master’s degree in Social Work (MSW) required
  • Minimum of 2 years post-graduate experience in a healthcare setting preferred
  • Experience in maternal-child health, pediatrics, adoption, or reproductive services preferred
  • Must be licensed by the Maryland Board of Social Work Examiners at the LMSW level
  • LCSW-C preferred

Responsibilities

  • Provides comprehensive assessment of patients to evaluate mental health, substance use, emotional functioning, coping style, and understanding of illness, barriers to care, cultural considerations, and risk factors such as abuse, neglect, and domestic violence.
  • Conducts psychosocial assessments of families to evaluate family systems, relationships, decision-makers, caregiving capacity, and overall understanding of the patient’s condition and care needs.
  • Facilitates and supports adoption and surrogacy processes within the hospital setting, including: Coordinating with adoption agencies, legal teams, and intended parents, Supporting birth parents and intended parents through emotional, ethical, and logistical aspects of the process, Ensuring compliance with hospital policies and applicable laws, Assisting with discharge planning specific to adoption or surrogacy arrangements.
  • Reports suspected abuse or neglect to legally mandated authorities in accordance with state regulations. Works closely with CPS to ensure safe discharge planning.
  • Employs a range of clinical interventions including crisis intervention, care coordination, information gathering, counseling, and referrals. Advocates on behalf of patients and families to ensure access to appropriate services and resources.
  • Coordinates family and interdisciplinary team meetings as appropriate. Provides psychosocial consultation on patient care planning, family dynamics, and utilization of community resources.
  • Identifies psychosocial, financial, educational, psychiatric, and substance use factors impacting care management and collaborates with the healthcare team to address these needs.
  • Works effectively as part of the interdisciplinary healthcare team, maintaining consistent communication and participating in case reviews, supervision, and educational opportunities.
  • Documents timely, accurate, and relevant information in the medical record.
  • Maintains up-to-date knowledge of internal and community-based resources and promotes their effective use.
  • Develops and maintains collaborative relationships with community collaborates, including adoption agencies, surrogacy programs, public health organizations, and social service providers.
  • Assist with task supervision of maternal child MSW interns.
