About The Position

Join our compassionate ICU team at Trinity Health Grand Rapids Hospital, where your skills in clinical social work will make a critical impact during some of the most vulnerable moments in our patients’ lives. In this fast-paced environment, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your work will help patients navigate crisis situations, overcome barriers, and access needed community resources to ensure safe and effective care transitions.

Requirements

  • Master’s Degree in Social Work or a related field from an accredited institution.
  • Eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
  • Minimum of one year of social work experience in a healthcare setting; experience in emergency or acute care preferred.
  • Exceptional crisis intervention and psychosocial assessment skills.
  • Strong emotional resilience and sound decision-making under pressure.
  • Compassionate communication and active listening.
  • Collaborative team player who thrives in a dynamic, high-acuity environment.
  • Familiarity with local resources and referral processes for at-risk populations.
  • Accurate, timely clinical documentation and case tracking.

Responsibilities

  • Provide social work services during the 8:00 AM – 4:30 PM shift in the ICU.
  • Conduct thorough psychosocial assessments for patients and families in crisis.
  • Develop individualized treatment and discharge plans that address emotional, social, and practical needs.
  • Deliver brief counseling and crisis intervention.
  • Coordinate with community agencies and internal resources to support safe care transitions.
  • Serve as a patient advocate and liaison between care teams and families.
  • Document assessments, plans, interventions, and outcomes in a timely and accurate manner.
  • Collaborate with the interdisciplinary care team to support holistic patient care.
