About The Position
As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being. To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.
Requirements
- Current California LCSW license
- 1-3 years 2 years Health care setting Licensed Clinical Social Worker: CA, upon hire
- Basic Life Support - CPR, upon hire
- Driver's License: CA, upon hire
- Auto Insurance, upon hire
Responsibilities
- Conduct psychosocial assessments
- Develop care plans
- Provide counseling and crisis intervention
- Connect clients to resources
- Advocate for clients
- Collaborate with multidisciplinary teams to ensure holistic support and promote well-being
- Meets discipline specific productivity standard
- Demonstrates competency in social work with hospice patients/families during field observation and testing
- Completes and submits documentation following established guidelines as measured by a defined evaluation process
- Physician orders are written according to policy
- Documents follow-up of identified problems
- Reviews and documents clinical progress toward goals
