This position provides assessment of patients/families and their adaptation to the patient's illness. It involves providing counseling as needed and ensuring maximum patient participation in plans for discharge and recovery. The role identifies system or discharge problems and develops discharge plans, presenting options to patients. It also includes the assessment of all potential cases of abuse: child, elderly, disabled and/or domestic violence.
