About The Position

The Clinical Social Worker (MSW) provides direct patient and family care and helps coordinate team-delivered services. The role supports patients and families with complex medical, psychosocial, and discharge-planning needs, partnering with interdisciplinary teams to support safe, timely, and patient-centered transitions of care.
