About The Position

Family Health Centers of San Diego (FHCSD) is seeking a Clinical Site Manager for Physical Rehabilitation Services to provide operational management, including administrative and clinical oversight, to Physical Rehabilitation Services and activities at assigned clinics. This role requires dedicating 60% of time to direct patient care as a Physical Rehabilitation Services clinician. The ideal candidate will demonstrate passion for providing high-quality care to a diverse patient population, with a special commitment to the uninsured, low-income, and medically underserved. FHCSD is a large federally qualified health center (FQHC) with a long-standing mission to provide comprehensive healthcare services to all, especially those in need.

Requirements

  • Master’s Degree of Physical Therapy, Chiropractic, or Acupuncture required.
  • Current California state clinical licensure as Physical Therapist, Chiropractor, or Acupuncturist.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
  • Possession of First Aid/CPR certificate issued by an authorized agency.
  • 3 years leadership experience in Healthcare.
  • 2 years experience leading or supervising in a Clinical setting required.
  • Ability to maintain good interpersonal interactions with clients and co-workers as a member of a multi-disciplinary community health team with a diverse multi-cultural population.
  • Traveling between sites and other locations is required as an essential function of the job.
  • Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law (limits of $100,000 recommended).
  • An acceptable driving record is also required.
  • Demonstrated commitment to community medicine and providing culturally competent care to the medically underserved.

Responsibilities

  • Provide operational management, including administrative and clinical oversight to Physical Rehabilitation Services and activities at assigned clinics.
  • Dedicate 60% of time to providing direct patient care as a Physical Rehabilitation Services clinician.
  • Build positive relationships, inspire staff to perform at a higher level, prepare career development plans, and retain an accountable workforce.
  • Coach and counsel staff and administer appropriate disciplinary action in a timely manner.
  • Ensure staff receive all new hire and annual trainings on time.
  • Administer day-to-day operations of assigned clinics and/or programs, including scheduling and patient service management.
  • Establish operational objectives and develop and implement plans for meeting tactical and strategic objectives as reviewed by senior management.
  • Oversee administration of integrated care activities at assigned clinics, including data collection and report generation.
  • Track site productivity and utilization rates.
  • Monitor ongoing revenue and expenses.
  • Collect and analyze data, prepare necessary reports, and develop improvement plans as necessary.
  • Provide direct patient services as appropriate within scope of licensure.
  • Actively participate in organizational short and long-term planning and implementation activities in collaboration with the Physical Rehabilitation Services Senior Management Team.
  • Oversee and directly participate in project planning, implementation, and assessment at assigned clinics.
  • Complete all required documentation accurately, in a timely and thorough manner, in accordance with department standards.
  • Ensure documentation complies with patients' insurance requirements and monitor documentation of direct reports and assigned clinics to ensure maximum revenue realization.
  • Work effectively as a member of a multi-disciplinary treatment team.
  • Communicate effectively with providers, physicians, case managers, scheduling staff, and department leadership in a timely and appropriate manner.
  • Review and maintain clinical guidelines.
  • Assess quality of clinical care and participate in clinical peer review.
  • Advise on standards of care for clinical practice.
  • Ensure applicable regulatory and department standards are met or exceeded on an ongoing basis (e.g., JCAHO, OSHA, Safety Standards).
  • Ensure assigned clinics and/or programs are managed in a fiscally responsible manner, including grants implementation and requirements, in collaboration with other FHCSD departments and clinics.
  • Ensure assigned site is well maintained and in compliance with FHCSD policies.
  • Work collaboratively with other managers in addressing and servicing difficult patients.
  • Ensure Physical Rehabilitation Services staff are kept informed and adhere to policies and procedures related to the Physical Rehabilitation Services clinic.
  • Participate in continuous improvement activities and ensure appropriate Lean tools (e.g., 5S, A3) are deployed and results are evaluated and/or measured.
  • Perform other duties as assigned.
