About The Position

The Clinical Services Specialist assists the Clinical Manager with the coordination and supervision of the day-to-day patient care operations of the Home Care department, including personnel, quality of care, and compliance with Hospice Savannah's policies and applicable state and federal regulations. This role is integral to ensuring seamless care delivery and operational efficiency across teams and facilities. The Office Nurse is committed to the philosophy, goals, and objectives of Hospice Savannah and consistently models our core values and service excellence standards.

Requirements

• Graduate of an accredited school of Nursing.
• Licensed Practical Nurse (LPN) license required.
• CPR certification required.
• Valid driver's license, automobile insurance, and reliable transportation required.
• Strong communication, leadership, customer service, and critical thinking skills required.
• Proficiency in Microsoft Office and database systems required.
• Mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics.

Nice To Haves

• Experience in hospice or home health preferred.
• Knowledge of Joint Commission standards and hospice-specific regulations preferred.

Responsibilities

• Ensuring seamless care delivery and operational efficiency across teams and facilities.
• Accurately, timely, and concisely document the patient/family encounter using a computer.
