About The Position

The Department of Family Medicine at South Waterfront is a busy primary care clinic providing comprehensive, lifespan care. The clinic offers services including general family medicine, transgender health, maternity and prenatal care, pediatric care, behavioral health, sports medicine, medication-assisted treatment, podiatry, acupuncture, minor procedures, and preventive health. Clinical care involves medication administration, triage, care coordination, patient education, complex wound care, pain management, and behavioral health support. This position serves a diverse patient population with a wide range of needs, from routine preventive care to complex chronic conditions, often influenced by social determinants of health.

Requirements

  • BSN Graduates: Baccalaureate Degree in Nursing from an accredited program (CCNE, ACEN, or CNEA) 30 days before start date.
  • ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before start date.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire.
  • Current, unencumbered Oregon State Registered Nurse License.
  • Active Washington RN License within 6 months of hire.
  • BLS from AHA required. New hires will be enrolled and required to complete during orientation.
  • Must be able to perform the essential functions of the position with or without accommodation.

Nice To Haves

  • Experience in independent RN triage via phone and in-basket messaging utilizing protocols.
  • Experience conducting RN led visits within scope (e.g., IV placements, medication administration, chronic disease education, and wound care).
  • Experience in longitudinal care management and care coordination.
  • Current Basic Life Support (BLS) issued by American Heart Association (AHA).

Responsibilities

  • Perform clinical triage and patient advice.
  • Provide care coordination and transitions of care.
  • Manage chronic diseases.
  • Engage in preventive care and population health initiatives.
  • Provide visit-based nursing care.
  • Educate patients and support self-management.
  • Administer and manage medications.
  • Conduct independent RN triage via phone and in-basket messaging utilizing protocols.
  • Conduct RN led visits within scope (e.g., IV placements, medication administration, chronic disease education, and wound care).
  • Engage in longitudinal care management and care coordination.
