Under the direction of the Clinical RN Manager, the Clinical RN Scheduling Coordinator will promote efficient client care, coordinate essential healthcare services, and uphold agency clinical compliance. This role will be responsible for performing the following functions to ensure the highest client care, professional streamlined coordination of essential clinical services and upholding agency clinical compliance standards:

Requirements

  • A minimum of 1-2 years Home Care experience, working in a clinical setting is a plus
  • HHAeXchange experience preferred
  • Experience and proficiency in navigating electronic health record (EHR) systems
  • High School Diploma or equivalent required; with a minimum of two years of administrative experience in a healthcare setting
  • Strong working experience using Microsoft Office Suite, including Excel, Word, and Outlook
  • Bilingual proficiency in English and Spanish is preferred
  • Must be extremely detailed orientated to ensure accuracy in addressing client needs, resolve issues, and create a positive experience.
  • Must have a high level of customer service, patience and problem-solving skills.
  • Strong interpersonal and communication skills to foster collaboration across departments.
  • Must have the ability to prioritize work tasks and responsibilities to get things done effectively.

Nice To Haves

Responsibilities

  • Monitoring interdepartmental communications to identify cases in need of clinical assessments
  • Running client reports to identify cases in need of 6-month recertifications assessments
  • Clinical visit assignment based on client demographic information
  • Facilitate visit scheduling between RN and client and troubleshoot when needed
  • Report concerns to management
  • Ensure all assigned visits are completed within compliance timeframe
  • Interact effectively with clients and nursing staff regarding their individual clinical needs, fostering professional relationships through clear, compassionate, and timely communication.
  • Support departmental operations by assisting with special projects, administrative tasks, and operational duties, as assigned by leadership, with responsibilities evolving based on organizational needs.
  • Maintain client records in accordance with HIPAA regulations, ensuring accuracy and compliance.
  • Serve as a liaison between field nursing staff, the Clinical Department, and other departmental directors as needed.
