Requirements

  • Active Registered Nurse (RN) license in the State of Maryland.
  • Minimum of three years of experience in skilled nursing, rehabilitation, or long-term care.
  • Strong clinical assessment, organizational, communication, and documentation skills.
  • Demonstrate Knowledge of CMS regulations, skilled nursing reimbursement, wound care, infection prevention principles, and quality improvement.
  • Experience with chart audits, admissions, and discharges.
  • Familiarity with electronic medical records (Point Click Care).

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred.
  • Wound care certification or specialized training preferred.

Responsibilities

  • Coordinate and complete resident admissions, including assessments and documentation.
  • Review admission paperwork for accuracy, completeness, and regulatory compliance.
  • Initiate care plans, verify physician orders, and ensure medication reconciliation.
  • Manage resident discharges, including final assessments, discharge documentation, and coordination with the interdisciplinary team to facilitate accurate transition of care.
  • Collaborate with nursing team members, therapists, social workers, to facilitate safe admissions and discharges.
  • Assist with survey readiness activities and monitor compliance with federal, state, and organizational standards.
  • Participate in quality assurance and performance improvement (QAPI) initiatives.
  • Conduct focused audits and track performance improvement metrics.
  • Assist with implementation and monitoring of evidence-based clinical protocols.
  • Conduct routine and focused chart audits to ensure accurate and timely documentation.
  • Identify documentation gaps and clinical concerns; communicate findings to nursing leadership.
  • Contribution to Keswick’s mission of improving the experience of aging through exceptional clinical care.
  • Review laboratory results promptly.
  • Identify abnormal or critical values and notify providers per facility protocol.
  • Ensure appropriate follow‑up, interventions, and documentation.
  • Monitor lab trends and support ongoing resident assessment.
  • Round with the Wound Care Physician and interdisciplinary team to ensure timely wound assessments, treatment implementation, and documentation.
  • Follow up on wound care recommendations and communicate changes to the interdisciplinary team.
  • Identify changes in resident condition and communicate concerns to the appropriate provider and nursing leadership.
  • Support nursing staff in implementing individualized resident care plans.
  • Ensure accurate and complete wound documentation, including measurements, staging, and progress.
  • Educate nursing staff on wound care procedures and documentation standards.
  • Serve as a clinical resource for nursing staff related to assessments, treatments, and documentation.
  • Guide best practices and facility policies in daily clinical operations.
  • Assist with clinical investigations and follow-up as assigned.
  • Promote teamwork, accountability, and a culture of continuous improvement.
  • Support nursing leadership with daily clinical operations as needed.
  • Perform other related duties as required.
