About The Position

RD Nutrition Consultants LLC is excited to offer an opportunity for a Clinical Registered Dietitian to join our team. This role allows flexibility and a balanced blend of in-person and remote work, ensuring our patients receive the best care while our team enjoys a work-life harmony. This position is for a Long-Term Acute Care facility and involves 24-30 hours per week, with 50% of the work completed remotely via telehealth. The schedule offers flexibility in choosing days and hours, with a requirement of two onsite visits per week.

Requirements

  • Bachelor’s or Master’s degree in Dietetics, Nutrition, or a closely related field from an accredited institution.
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration (CDR).
  • Current and valid state licensure or certification, where applicable.
  • Exceptional verbal and written communication skills for effective, empathetic interaction with patients, families, and multidisciplinary healthcare teams.
  • Demonstrated ability to apply critical clinical judgment to diverse patient scenarios, evaluate objectives, and implement effective performance improvement plans.
  • Proficiency in designing and executing targeted clinical nutrition interventions.
  • Familiarity with state survey processes and direct experience adhering to strict healthcare nutrition regulations.
  • Competency with electronic charting and electronic medical record platforms.

  • Minimum of 1 year of clinical nutrition experience preferred.

Responsibilities

  • Conduct comprehensive nutritional assessments on new admissions, perform scheduled quarterly reviews, and provide high-frequency monitoring for high-risk residents.
  • Develop, implement, and monitor individualized nutrition care plans tailored to resident medical needs and therapeutic goals.
  • Provide tailored nutrition education and dietary counseling to residents, family members, and care partners.
  • Partner seamlessly with physicians, nursing staff, therapists, and the broader interdisciplinary care team to integrate nutrition into holistic patient care.
  • Ensure strict compliance with all state and federal healthcare regulations, specifically Centers for Medicare & Medicaid Services (CMS) and Department of Health (DOH) mandates.
  • Maintain meticulous, timely, and compliant clinical documentation within electronic health records systems.
  • Track and analyze resident weight changes on a weekly or monthly basis to proactively identify and address shifts in nutrition or hydration status.
  • Actively participate in clinical quality improvement initiatives to optimize nutritional care services and resident outcomes.
  • Conduct monthly kitchen and food service sanitation audits, documenting findings and recommending immediate corrective actions to ensure a safe, sanitary environment.
  • Review and approve facility menus on a semi-annual basis to ensure full compliance with USDA dietary guidelines.
  • Execute additional professional duties and operational tasks as assigned within the established scope of practice.
