LV HOME CARE AND HOSPICE CLINICAL QUALITY AND EDUCATION SPECIALIST - HOSPICE Summary Conducts hospice quality improvement and regulatory readiness activities while serving as a clinical education resource and role model for hospice clinicians. Performs hospice QAPI activities, utilizing a data-driven approach to realize improvements in outcomes and patient experience. Works as a member of the Home Health Education team to design, deliver, and evaluate clinical education in didactic, simulation, and field environments to strengthen documentation, clinical practice, symptom management, and caregiver training.

Requirements

  • Bachelor’s Degree Nursing
  • 3 years Recent experience in hospice or palliative care, with demonstrated leadership and/or teaching ability
  • Knowledge and competence that reflects current nursing practice.
  • Ability to identify clinical problems and review related information to develop and evaluate options and implement solutions/quality care interventions.
  • Intermediate knowledge of evidence-based practice.
  • Intermediate computer skills; working knowledge of Excel for analysis and improvement (pivot tables, statistical tools), applications for data visualization and project facilitation.
  • BLS - Basic Life Support - American Heart Association
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania

  • CHPN - Certified Hospice and Palliative Nurse - Hospice and Palliative Nurses Association Certification in hospice care (CHPN) OR healthcare quality (NAHQ Performance and Process Improvement micro-credential) OR IHI Basic Certificate in Quality and Safety within 2 years

Responsibilities

  • Facilitates day-to-day function of the hospice QAPI program: conducts audits, collects and analyzes data, and facilitates improvement teams / projects.
  • Monitors compliance with HQRP, including HOPE assessments, Visits in Last Days of Life, and Hospice CAHPS.
  • Partners with hospice leaders to set targets and action plans for process and outcome improvements.
  • Applies standardized improvement and change management tools and techniques (PDSA, process mapping, etc.) to ensure continuous improvement and integration of evidence-based best practices.
  • Gathers and analyzes hospice process and outcome data from EHR and other sources, monitors for trends, and presents information to clinicians.
  • Participates in planning and delivery of orientation and continuing education to hospice clinical staff.
  • Teaches the HOPE data set including item intent, effective assessment techniques, and methods for compliance.
  • Coaches clinical staff on evidence-based best practices for symptom control, caregiver teaching and communication, and safe medication management.
  • Provides individualized education for accurate real-time documentation.
  • Serves as a clinical resource in didactic, simulation, and bedside environments to reinforce standards and ensure integration of best practices.
  • Precepts clinical staff as needed.
  • Monitors industry resources and network committees/councils for practice changes and disseminates to clinical staff.
  • Provides real-time support and advice to field staff when clinical questions arise.
