About The Position

Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.

Requirements

• Bachelor’s degree or equivalent Required.
• 2 years of relevant experience required
• LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire Required Or LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire Required Or CRT-Registered Dietitian (RD) – CDR Commission on Dietetic Registration Upon Hire Required Or LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan Upon Hire Required Or LIC-Physical Therapist - STATE_MI State of Michigan Upon Hire Required Or CRT-Occupational Therapist, registered (OTR) - NB-COT National Board for Certification in Occupational Therapy Upon Hire Required Or CRT-Speech Language Pathologist – ASHA American Speech-Language-Hearing Association Upon Hire Required Or CRT-Nuclear Medicine Technologist – NMTCB Nuclear Medicine Technology Certification Board Cardiovascular Dept.
