About The Position

Under the direction of Clinical Pharmacy Leadership and Clinical Pharmacist, the Clinical Program Assistant will perform assigned tasks necessary to support comprehensive clinical pharmacy services and disease management programs provided by the Clinical Pharmacists. The Clinical Program Assistant will be organizing and collecting clinical information for the clinical pharmacy appointments and medication reconciliation process. Perform chart reviews to identify patients who may benefit from clinical pharmacy services based on a combination of medical and pharmacy triggers. Contact patients by phone to explain the purpose of the clinical pharmacy service and record information from the patient regarding their medications in preparation for the Clinical Pharmacists to review. Ensure patients leave clinical pharmacy visits with appropriate education materials, follow-up plans, and return appointments. Collect health outcome data and assemble reports as needed. Requirements

  • Within 2 years of hire: Pharmacy Technician Certification
  • 2 years of Healthcare experience in a clinic, retail pharmacy or other practice environment involving patient interaction.

Nice To Haves

  • Successful completion of a pharmacy technician program
  • Upon hire: Pharmacy Technician Certification Board Certification
  • Experience working in Electronic medical records, preferably EPIC.

Responsibilities
