About The Position

The Clinical Pharmacy Specialist at John Muir Health drives the enterprise clinical pharmacy program and leads system initiatives to improve medication use, quality, and clinical performance. Each specialist focuses on a primary program area such as drug diversion prevention, medication safety, antimicrobial stewardship, formulary management, or clinical informatics, and provides cross-coverage for other areas. The role involves developing protocols, dashboards, and key performance indicators; leading committees or workgroups; monitoring regulatory compliance; benchmarking performance; and analyzing clinical data to evaluate therapeutic interventions. This position is crucial for executing the pharmacy clinical quality plan and achieving the ASHP Center of Excellence in Medication Management designation. The specialist also provides education, serves as a preceptor for pharmacy students and residents, and collaborates with medical staff, nursing, quality, and compliance leaders to enhance medication use, safety, and outcomes. This is a system-focused quality role, not primarily a patient-facing position, with direct patient care provided only as needed to support program objectives. The specialist may also cover clinical or operational shifts in pharmacy areas to ensure safe staffing and practice competency.

Requirements

  • Doctor of Pharmacy (PharmD from a school accredited by ACPE) - Required
  • At least 2 years of clinical pharmacy experience - Required
  • Completion of ASHP accredited PGY1 residency program OR 3 or more years of full-time, post-graduate clinical pharmacy experience in an acute care or specialized health-system setting - Required
  • RPH Registered Pharmacist - California Board of Pharmacy - Required
  • Board Certification in a pharmacy specialty recognized by the Board of Pharmacy Specialties - Required Or obtained within 9 months of hire.
  • Proven ability to multitask and adapt to a high-acuity inpatient and ambulatory environment.
  • Demonstrated ability to perform in a complex and collaborative clinical setting and drive results.
  • Experience in developing policies or workflows and engaging in continuous process improvement.
  • Must have the ability to work all shifts, including weekends.

Nice To Haves

  • Completion of ASHP certified PGY2 residency program in relevant specialty.
  • Demonstrated SME expertise in one or more of the following program areas — drug diversion prevention, medication safety/medication-use systems, antimicrobial stewardship, formulary management and drug policy, or clinical informatics — with willingness and ability to cross-cover and support.
  • Experience leading or supporting an effective controlled substance diversion prevention program, including surveillance, investigation, and interdisciplinary reporting.
  • Experience with leading medication event reporting and analysis, root cause analysis, and medication-use system improvement with proven results.
  • Experience with antimicrobial stewardship interventions, formulary management, and reporting to meet CMS and Joint Commission requirements with proven results.
  • Advanced Pharmacist Practice License (APh) - Preferred

  • Provides medication and practice-related education.
  • Serves as a preceptor for pharmacy students and residents.
  • Partners with medical staff, nursing, quality, and compliance leaders to improve medication use, safety, and outcomes.
  • Provides direct patient care as needed to advance program objectives and validate practice.
  • May be scheduled to cover clinical or operational shifts in pharmacy areas as needed to maintain safe staffing, continuity of service, and practice competency.
