About The Position

This Cardiovascular Services specialist role offers an exceptional opportunity to directly influence patient outcomes, medication safety, discharge readiness, and readmission reduction across the cardiovascular inpatient episode. The specialist will play a critical role in optimizing medication continuity and transitions of care for high‑risk cardiovascular patients as they move from procedural areas through the CVICU/CVRR, S7B Cardiothoracic Stepdown Unit, and ultimately to discharge.

Requirements

  • Doctor of Pharmacy (PharmD)
  • PGY‑2 Critical Care Pharmacy Residency with significant cardiology experience
  • Florida pharmacy licensure eligibility required

Nice To Haves

  • PGY‑2 Cardiology Pharmacy Residency preferred
  • Board Certification in Cardiology preferred or eligibility to sit within 3 years required

Responsibilities

  • Participate in multidisciplinary rounds for high‑risk cardiovascular patients in the CVICU/CVRR and S7B Cardiothoracic Stepdown Unit
  • Manage medication continuity across transitions: procedural areas → CVICU/CVRR → stepdown → discharge
  • Optimize high‑risk cardiovascular therapies including anticoagulants, antiplatelets, antiarrhythmics, vasoactive agents, heart failure medications, and post‑surgical regimens
  • Support discharge readiness through medication reconciliation, patient/caregiver education, affordability solutions, and clear communication of final medication plans
  • Provide consultative support to Cath Lab and Electrophysiology leaders; develop and refine medication‑use protocols, pathways, guidelines, and order sets
  • Identify and implement cardiovascular medication‑use, quality, and safety initiatives to reduce variation, discharge barriers, and avoidable readmissions
  • Collaborate with cardiovascular surgery, cardiology, nursing, case management, and pharmacy operations to strengthen medication handoffs
  • Educate and serve as a clinical resource for physicians, nurses, pharmacists, residents, fellows, and learners
  • Precept and mentor PGY‑1 and PGY‑2 pharmacy residents and pharmacy students
