The Clinical Pharmacist reports to the Director of Pharmacy and is responsible for providing safe, effective, evidence-based medication therapy management in collaboration with physicians, nurses, pharmacy team members, and other interdisciplinary care partners. This position reviews, verifies, and monitors medication orders; evaluates drug therapy for appropriateness, safety, efficacy, interactions, allergies, contraindications, dosing, duplicate therapy, and patient-specific clinical factors; and supports timely, accurate dispensing of medications in accordance with organizational policies, regulatory standards, and applicable state and federal requirements. The Clinical Pharmacist serves as a medication therapy expert and resource to clinical teams, patients, and caregivers by providing drug information, patient education, medication reconciliation support, and recommendations that promote optimal patient outcomes. This position also supports day-to-day pharmacy operations, medication safety initiatives, quality improvement activities, inventory stewardship, documentation requirements, and the supervision and guidance of pharmacy support personnel and students.
