The Clinical Pharmacist will oversee pharmaceutical care within the Inpatient Pharmacy at Carolinas Medical Center. This role involves retrieving and evaluating biomedical literature, designing medication therapy regimens, and utilizing effective patient education techniques to counsel patients and caregivers. The pharmacist will design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens, implementing recommendations and documenting them appropriately. Responsibilities also include reviewing medication orders, participating in medical emergencies, supporting medication safety and quality improvement activities, and performing quality assurance of the medication-use system. Collaboration with the healthcare team on formulary adherence and medication-use processes is essential, as is assisting with the acquisition of specialized medications. The role requires practice in compliance with organizational policies and all relevant regulations, including the preparation, dispensing, and proper storage of medications. Additionally, the Clinical Pharmacist will serve as a preceptor for pharmacy students and residents, mentor and train pharmacy teammates, and direct the work of pharmacy support staff to ensure safe and efficient workflows. The position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan and assessing patient needs based on age-specific requirements.
