About The Position

Sentara Carplex Hospital is hiring a Clinical Pharmacist for their Inpatient Pharmacy team in Hampton, VA. This is a Full Time position (36 hrs/wk), requiring nine 8-hour shifts per pay period, rotating days and evenings with occasional overnight coverage. The Clinical Pharmacist is responsible and accountable for providing safe, effective, and prompt pharmacy services, participating in all necessary aspects of the medication-use system, and delivering comprehensive and individualized pharmaceutical care to patients in assigned areas. The role involves managing patient drug therapy, ensuring appropriateness of medication orders, maintaining current knowledge of medication use and disease states, serving as a drug information resource, providing patient counseling, and participating in employee training and student preceptorship. The position requires superior customer service, effective communication, decision-making, and organization, and involves ensuring adequate supplies, triaging issues, using appropriate systems for patient information, maintaining quality assurance, and operating pharmacy equipment.

Requirements

• Bachelor's Level Degree in Pharmacy
• Virginia Pharmacist License
• Basic Life Support (BLS)

Nice To Haves

• Doctorate Level Degree in Pharmacy
• Advanced Cardiac Life Support (ACLS)
• Previous Clinical Experience
• PGY1/PGY2 Residency Trained

Responsibilities

• Participates in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas
• Demonstrates the ability to work in areas of pharmacy practice competently and independently
• Manages patient drug therapy in accordance with applicable clinical practice standards
• Assures the appropriateness of all provider medication orders in maintaining a high level of patient care and safety
• Maintains current knowledge of medication use and disease states applicable to their area of practice
• Serves as the primary resource for drug information and education
• Provides in-depth patient counseling when required
• Participates in the training of new employees as assigned
• Assumes local responsibility and ownership for assigned tasks
• Consistently follows policies and procedures
• Continually seeks out additional training opportunities and stays abreast of new and changing information for assigned areas
• Demonstrates superior customer service, effective communication, decision-making, and organization to ensure efficient job performance and success
• Serves as a role model in the following areas: professionalism, collaboration, and teamwork
• Ensures adequate supplies are available for continuous patient care
• Triages and escalates any questions or issues outside the scope of practice as needed

• Ensures quality assurance is maintained according to departmental and legal requirements
• Operates pharmacy equipment appropriately
• Precepts pharmacy students as assigned
