About The Position

A reputable healthcare organization is seeking a Clinical Pharmacist to provide comprehensive clinical and pharmacy services in an acute care setting. This role includes overseeing pharmacy operations, supporting technical staff, and collaborating with a multidisciplinary team to ensure safe, effective, and evidence-based medication use.

Requirements

• Minimum 1 year of recent Clinical Pharmacist experience in an acute care hospital (within the last year)
• Bachelor’s (B.S.) or PharmD from an ACPE-accredited School of Pharmacy
• Active Florida Pharmacist License in good standing

Responsibilities

• Accurately interpret and verify medication orders in the pharmacy system
• Compound and dispense medications safely and efficiently
• Monitor drug therapy for contraindications, interactions (drug-drug, drug-food), allergies, and appropriate dosing
• Maintain hospital formulary and promote evidence-based medication use
• Provide drug information and counseling to healthcare staff and patients
• Participate in clinical pharmacy services, including: Drug consultations, Nutrition support, Pharmacokinetics, Anticoagulation monitoring, Pain management, IV-to-PO conversions, Renal dosing adjustments
• Document clinical interventions thoroughly
• Report medication errors and adverse drug reactions
• Supervise pharmacy operations and support pharmacy staff
