About The Position

High Desert Medical Group is seeking a full time Clinical Pharmacist to work in our Disease Management Clinic. The Pharmacist will be responsible for providing clinical pharmaceutical care to patients, and for interacting with physicians/health care professionals regarding Disease Management Programs and medical reconciliation.

Requirements

  • Doctorate in Pharmacy required.
  • Ability to follow and adhere to FDA Health Regulations, and other legal compliances.
  • Excellent written and verbal communication skills.
  • Ability to communicate effectively with patient and medical team members.
  • Ability to handle and prioritize tasks in order to meet all given deadlines and productivity goals.
  • Ability to responsibly handle matters of a confidential nature.
  • Ability to work in a multi-task, high volume environment.

Nice To Haves

  • Hospital Pharmacy Residency or comparable pharmacy experience preferred.

Responsibilities

  • Comply with state and federal drug laws as regulated by the California State Board of Pharmacy, the Drug Enforcement Administration, and the Food and Drug Administration.
  • Medication reconciliation.
  • Perform Disease Management Programs duties, including but not limited to contacting the patient(s) medication dosage titrations, and diabetic panel parameters assessment.
  • Consult patients in hour and IPA physicians on matters concerning diabetes.
  • Communicate with the patients for medications counseling and for treatment regiments, including but not limited to diabetes, COPD, CHF, ESRD, Anticoagulation, Osteoarthritis and other disease conditions.
  • Monitor patients for side effects and adverse events, and maintain communication with the physicians and nurses.
  • Provide other non-drug recommendations, including weight management and diet counseling.
  • Keep an accurate journal and review clinical studies relevant to the pharmacy practice at HDMG.
  • Present data and perform presentations for the house and IPA physicians.
  • Effectively verify patient medication orders for appropriateness and accuracy.
  • Actively participate in HDMG alert response system when necessary.
  • Actively document clinical interventions and recommendations.
  • Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
  • Provide and document discharge medication teaching upon request.
  • Support the mission, vision, and values of the Pharmacy Department at HDMG, along with the department's policies and procedures.
  • Participate in the clinical guidelines' development, critical pathways, disease management and drug use programs as identified and under the mentorship of the Pharmacy Supervisor/Director.
