The Clinical Pharmacist - Night Shift position at Carolinas Medical Center - Pharmacy is a full-time role responsible for overseeing pharmaceutical care. This includes retrieving and evaluating biomedical literature, designing medication therapy regimens, and using effective patient education techniques for counseling patients and caregivers. The role involves designing, recommending, monitoring, and evaluating patient-specific pharmacotherapeutic regimens, implementing recommendations, and documenting in the appropriate format. The pharmacist will review and interpret medication orders and prescriptions, participate in medical emergencies, and actively support medication safety and quality improvement activities. They will also perform quality assurance and performance improvement of the medication use system, collaborate with the healthcare team on formulary adherence and medication-use processes, and assist in acquiring necessary medications. Practice must comply with organizational policies, procedures, and local, state, and federal regulations. The role also includes preparing, dispensing, and overseeing the proper storage and handling of medications. Additionally, the Clinical Pharmacist will serve as a preceptor for pharmacy students, residents, and healthcare learners, and mentor, orient, train, and assist with performance evaluations of pharmacy teammates. They will direct and check the accuracy of pharmacy support team members' work to ensure safe and efficient workflows and regulatory compliance. The position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development principles and the ability to assess and interpret patient data based on age-specific needs.
