About The Position
This role involves developing competency in clinical pharmacy practice, including appropriate drug therapy for patients and the prevention/resolution of drug-related problems. The Clinical Pharmacist will evaluate patients for drug interactions, individualize drug therapy regimens, and facilitate medication-related continuity of care. Responsibilities also include reviewing and interpreting medication orders, dispensing and compounding medications according to regulations, ensuring formulary adherence, recommending cost-effective therapies, and minimizing waste. The position actively supports initiatives to prevent medication events and enhance medication safety and quality improvement. Additionally, the role involves orienting and training new pharmacy team members, potentially guiding interns, residents, and students, and providing direction to pharmacy technicians.
Requirements
- Pharmacist license issued by the state in which the team member practices.
- Bachelor's Degree in Pharmacy.
- No experience required.
- Maintains skills in area of practice.
- Strong computer skills.
- Excellent communication and organization skills.
- Excellent presentation, facilitation and training development skills.
- Demonstrated decision making, team building, customer service, and conflict resolution skills.
Responsibilities
- Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems.
- Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions.
- Individualizes drug therapy regimens based on patient’s size, organ functions, and disease states.
- Facilitates medication-related continuity of care at admission, transfer and discharge.
- Reviews and accurately interprets medication orders.
- Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling.
- Practices in compliance with department policies & procedures, as well as local, state and federal regulations.
- Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice.
- Actively supports procedures and initiatives to prevent medication events. Reports events through appropriate channels. Actively supports department medication safety and quality improvement activities.
- Orients/trains new pharmacy team members. May work with interns, residents and students to guide daily activities. May be preceptor-in-training. Provides direction to pharmacy technicians to ensure smooth workflow and service standards.
