About The Position

Responsible for reviewing physician medication orders (EPIC CPOE) for appropriateness and cost-effectiveness, monitoring and adjusting patient drug therapies according to pharmacy-driven protocols, and providing drug information to patients and other healthcare providers. Providing pharmaceutical care including anticoagulant, PK, renal dosing, IV to PO, insulin dosing, antimicrobial stewardship, and many other pharmacy protocols. Drug therapy management via established policies and procedures, ensuring that the pharmacy provides optimal services on a daily basis. Pharmacists will also be involved in quality improvement activities, distributive activities and overseeing techs, precepting PharmD students and PGY1 residents. In addition, may also be assigned other duties as required.

Requirements

  • Graduate of an ACPE-accredited School of Pharmacy.
  • Doctorate in Pharmacy (PharmD).
  • Completion of an ASHP accredited residency program or at least two (2) years of inpatient clinical experience.
  • ACLS certification.
  • Strong knowledge of computer systems/EPIC, Pyxis, barcoding and inpatient workflow.
  • Current CA Pharmacy license required.
  • Effective communication skills (written and verbal) in English.
  • Proficient with Microsoft Office applications.
  • Good planning and organizational skills.
  • Emotional stamina to work in a stressful environment.
  • Computer and barcode knowledge, including pharmacy operations.
  • Must be able to cover all shifts as needed.
  • Must be able to cover day, evening, or night shifts as needed, including weekends and holidays, and coverage for sick calls and vacations.

Nice To Haves

  • Cardiology and Oncology experiences are a plus.
  • Proficient with EPIC.
  • Cardiology, Oncology, Transition of Care experiences are preferred.
