About The Position

Responsible for reviewing physician medication orders (EPIC CPOE) for appropriateness and cost-effectiveness, monitoring and adjusting patient drug therapies according to pharmacy-driven protocols, and providing drug information to patients and other healthcare providers. This role involves providing pharmaceutical care including anticoagulant, PK, renal dosing, IV to PO, insulin dosing, antimicrobial stewardship, and other pharmacy protocols. The pharmacist will manage drug therapy via established policies and procedures, ensuring optimal pharmacy services daily. Responsibilities also include involvement in quality improvement activities, distributive activities, overseeing technicians, and precepting PharmD students and PGY1 residents. Additional duties may be assigned as required. Oncology experience is a plus.

Requirements

  • Graduate of an ACPE-accredited School of Pharmacy.
  • Doctorate in Pharmacy (PharmD).
  • Completion of an ASHP accredited residency program or at least two (2) years of inpatient clinical experience.
  • Current CA Pharmacy license required.
  • Effective communication skills (written and verbal) in English required.
  • Proficient with computer systems/EPIC, Pyxis, barcoding and inpatient workflow.
  • Must be able to cover all shifts as needed.

Nice To Haves

  • Oncology experiences are a plus.
  • ACLS certification desired.
  • Proficient with Microsoft Office applications.
  • Proficient with EPIC desired.
  • Good planning and organizational skills.
  • Emotional stamina to work in a stressful environment desired.
  • Oncology and Transition of Care experiences are preferred.
  • ASHP accredited Residency, ACLS, BLS preferred.
