About The Position

This position oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. The Clinical Pharmacist uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration. The role involves designing, recommending, monitoring, and evaluating patient-specific pharmacotherapeutic regimens based on relevant biomedical literature, and implementing recommendations effectively using policies and procedures. The pharmacist will review and accurately interpret medication orders and prescriptions, participate in the management of medical emergencies, and actively support department medication safety and quality improvement activities. Additionally, the role performs quality assurance and performance improvement of the medication-use system, collaborates with the healthcare team to manage formulary adherence and the medication-use process, and assists in the acquisition of non-formulary, compassionate use, and investigational medications. Practice must comply with organization’s policies and procedures, as well as local, state, and federal regulations. The pharmacist will prepare, dispense, and oversee the proper storage and handling of medications. This role also includes serving as a preceptor for pharmacy students, residents, and healthcare learners, mentoring, orienting, training, and assisting with performance evaluations of pharmacy teammates, residents, and students. The Clinical Pharmacist directs and checks the accuracy of the work of pharmacy support team members to ensure safe and efficient workflows, service standards, and regulatory compliance. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, including understanding of growth and development over the life span and the ability to assess data reflective of patient status and interpret information needed to identify age-specific needs.

Requirements

• Current state pharmacist license in good standing through North Carolina Board of Pharmacy or eligible for reciprocity is required.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.
• Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) is required.

• Assists in the acquisition of non-formulary, compassionate use, and investigational medications.
• Practices in compliance with organization’s policies and procedures, as well as local, state, and federal regulations.

• Directs and checks the accuracy of the work of pharmacy support team members in accordance with the organization’s policies and procedures to ensure safe and efficient workflows, service standards, and regulatory compliance.
• Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
• Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures.
