About The Position

Sentara Health Plans is hiring for a Clinical Pharmacist for the Health Plan- Remote! This is a full-time, permanent position (40 hours) with standard working hours of 8am to 5pm EST, Monday through Friday. The role involves demonstrating comprehensive knowledge of current pharmacotherapeutic principles, medical terminology, pathologies, disease state and population management, age-appropriate therapy, and other information related to medical and/or pharmaceutical care management. The Clinical Pharmacist will work with Sentara Health Plan (SHP) members to close medication-related gaps in care, handle referrals from the team, and counsel members on their medication therapy. Sentara Health Plans provides health plan coverage to close to one million members in Virginia, offering a full suite of commercial products, Individual & Family Health Plans, Employee Assistance Programs, and plans serving Medicare and Medicaid enrollees. Requirements

  • Bachelor’s degree in Nursing, Clinical Sciences, Population Health, Healthcare Administration, Business Administration, or related field
  • Pharmacist License (RPh) - State license
  • Experience with online pharmacy adjudication systems

Nice To Haves

  • Advanced certification in HIV, oncology, infectious disease, or ambulatory care preferred
  • Experience with HEDIS or Managed Care Pharmacy OR Pharmacy Benefits Management (PBM) preferred – highly preferred

Responsibilities

  • Work with Sentara Health Plan (SHP) members to close medication-related gaps in care
  • Work on the referrals from the team
