About The Position

Axil Health is seeking an exceptional Clinical Pharmacist to lead their Chronic Care Management (CCM), Transition Care Management (TCM), and Remote Patient Monitoring (RPM) services. This full clinical service program will operate through a Collaborative Practice Agreement with independent medical facilities across North Carolina. The role involves establishing and maintaining professional relationships with team members within medical practices. Responsibilities may vary depending on the unique needs of each independent practice. The role is performed in provider clinics and may require regional travel to a group of clinics. The pharmacist will build and work directly with a patient panel to drive clinical outcomes and results, spearhead patient enrollment, device management, and data interpretation for RPM, and collaborate on medication management with supervising providers. The role also includes participating in and leading topic discussions for current staff and residents, precepting PGY-1 Students, participating in the Fellowship Program, developing individualized care plans, and providing recommendations or implementing therapy changes.

Requirements

  • A current, valid pharmacist license to practice in North Carolina.
  • Current CPR certification.
  • Certification for Immunization.

Nice To Haves

  • PharmD Degree with three years of experience.
  • Currently, hold a Board of Pharmaceutical Specialty Certification or Completion of APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program.
  • Completion of an APhA Certificate Training Program.
