Endocrinology clinic-embedded pharmacist responsible for medication therapy for various chronic conditions including, but not limited to diabetes, thyroid conditions, osteoporosis, dyslipidemia, hypertension. Maintains updated collaborative practice agreement, as applicable Performs point-of-care tests according to CLIA guidelines and hospital regulations as well as ordering laboratory tests as clinically appropriate to monitor effectiveness of therapy (HbA1c and lipid panels). Assesses patient needs and provides tools to support the patient in their self-care. Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on relevant biomedical literature. Implements recommendations effectively and utilizes policies and procedures and documents in the appropriate format. Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration. Documents clinical activities in patients’ electronic health record to provide information needed to meet standards, regulations, laws and/or other administrative requirements Updates knowledge and skills of self and other health care professionals to maintain, improve and document competency. Participates in planning by establishing personal goals to support quality improvement efforts and contribute to the achievement of departmental objectives. Practices in compliance with organization’s policies and procedures, as well as local, state, and federal regulations. Serves as preceptor for pharmacy students, residents, and healthcare learners as assigned. Mentors, orients, trains, and assists with the performance evaluation of pharmacy teammates, residents, and students as assigned.
