About The Position

Work Schedule : 100% FTE, day shift. Monday – Friday; 8:00 AM – 4:30 PM. You will work at Junction Road Medical Center in Madison, WI with additional locations as needed and the opportunity for remote work. Hours may vary based on the operational needs of the department. We are seeking a Clinical Pharmacist to: Develop and launch pharmacy services with collaboration from the pharmacy manager and clinical team, with the goal of enhancing patient access, optimizing medication access, and improving patient outcomes. Assess patient understanding of medication regimen and provide education tailored to patients with diabetes, osteoporosis, and parathyroid disease. Provide guidance and support for medication access and affordability programs to reduce barriers to care. Provide training and education to pharmacy residents and PharmD students through practical and didactic experiences. The ideal candidate would be Board Certified in Advanced Diabetes Management (BC-ADM) or be a Certified Diabetes Care and Education Specialist (CDCES). Requirements

Bachelor's Degree in Pharmacy or a PharmD degree
1 year of clinical patient care and orders management experience
Licensed to practice pharmacy in the State of Wisconsin within 90 Days
Basic Life Support (BLS) within 90 Days

Nice To Haves

Completion of an ASHP Accredited Residency training program
3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program
Board Certified in Advanced Diabetes Management (BC-ADM) or be a Certified Diabetes Care and Education Specialist (CDCES).
Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.)
