The Clinical Pharmacist – Transplant, PRN provides decentralized clinical pharmacy services for the inpatient Lung Transplant Program, partnering closely with transplant physicians, advanced practice providers, nurses, and the multidisciplinary care team to optimize medication therapy for complex transplant patients. This role supports patients throughout the continuum of inpatient care, from the immediate post-transplant period in critical care through recovery on acute care units, with a strong focus on immunosuppressive therapy management, therapeutic drug monitoring, and individualized pharmacotherapy. As an integral member of the transplant team, the pharmacist participates in direct patient care, provides evidence-based medication recommendations, and helps ensure safe, effective medication use while supporting one of Cleveland Clinic's nationally recognized transplant programs. This is a PRN role. A caregiver in this position will work various shifts.

Requirements

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy
  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
  • Completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for individuals administering immunizations

  • Board certification
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience preferred.
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area

Responsibilities

  • Provide safe, efficient, patient-centered and evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers and/or patients.
