About The Position

Clinical Pharmacist within our Pulmonology Office works with the care team optimizing patient care by providing specialized pharmaceutical expertise and support. Working closely with the pulmonologists and the medical team, the Clinical Pharmacist contributes to the safe, effective, and efficient management of patients’ medication regimens. The pharmacist’s responsibilities encompass medication therapy management, patient education, and collaborative work in patient care plans.

Requirements

  • Current Michigan licensure to practice as a Pharmacist
  • Computer competency including but not limited to EPIC and Lotus notes
  • Ability to utilize all reference material needed to provide pharmaceutical care.
  • Has the training and abilities necessary to be a Clinical Pharmacist
  • Excellent oral and written communication skills.
  • Must be able to work well with all our customers and able to be tactful in often hectic and stressful situations.
  • Pharmacist skill set that emphasizes clinical activities
  • Sound judgment in handling confidential or sensitive issues and material.
  • Able to flex work schedule in order to meet department and staffing needs

Nice To Haves

  • PharmD preferred.
  • 1 yr. Residency preferred.
  • Maintains professional development through seminars, workshops, in-service activities, and current professional literature.
  • Minimum of 3 years experience in a hospital setting preferred
  • Previous clinical pharmacy experience preferred

Responsibilities

  • Assists physicians in disease management within institution approved collaborative practice agreements.
  • Staffing requirements will include reviewing prescriptions issued by the Physician, or other authorized prescriber to assure accuracy, appropriateness of medication, and determine formulas and ingredients needed.
  • Educate health care professionals using multiple communication methods, such as presentations, newsletters, emails, the pharmacy intranet, one-on-one sessions, and other means.
  • Provides education and information on pharmaceutical care and drug therapy to patients, pharmacy staff, physicians, nurses, students and other clinicians.
  • Provides drug therapy management and assessment including identifying and resolving potential and actual drug related problems.
  • Provide clinical recommendations pertaining to gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution.
  • Utilize formulary information to guide appropriate medication recommendations.
  • Keep current with new and emerging evidence-based medicine and therapeutic recommendations.
  • Participates in performance improvement initiatives and research as appropriate.
  • Efficiently perform evidence-based scientific evaluation of the medical literature evaluations using information from multiple databases and other sources.
  • Work with electronic medical record pharmacists to optimize the use of clinical decision support tools and order sets to improve medication use process and patient outcomes.
  • Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.
  • Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member or consultant.
  • Performs other related duties as assigned such as quality assurance reports, preparation for regulatory inspections, etc.
