About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Clinical Pharmacist to join the Pediatric Clinical Pharmacy Team at Monroe Carrell Jr. Children’s Hospital at Vanderbilt. This role primarily focuses on Neonatal ICU patients. The Clinical Pharmacist will provide direct patient care through multidisciplinary interactions, optimize pharmacotherapy, assist with discharge planning, verify orders, and respond to codes and bedside procedures. Collaboration with pharmacy, nursing, and medical staff, along with excellent communication skills, are essential. This position involves dayshift hours and requires staffing every 6th weekend. The role emphasizes leadership in specialized healthcare teams to ensure optimal, cost-effective drug therapy, including developing pharmacotherapeutic goals, recommending regimens, and creating monitoring plans. The clinical pharmacist is also responsible for leading efforts in medication error and adverse drug reaction prevention and reporting, contributing to continuous process and practice improvement to enhance medication therapy safety.

Requirements

  • LIC-Pharmacist License - Tennessee
  • Relevant Work Experience
  • 1 year of experience
  • Bachelor's degree

Nice To Haves

  • Completion of ASHP accredited PGY-1 residency program, in a pediatric hospital setting
  • 2 years of pediatric hospital experience

Responsibilities

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
