The Clinical Pharmacist provides leadership in a specialized healthcare team to ensure optimal cost-effective drug therapy. This involves developing specific pharmacotherapeutic goals, recommending regimens, and creating monitoring plans for individualized drug therapy. The role also includes leading efforts in preventing and reporting medication errors and adverse drug reactions, and continuously improving processes to enhance medication therapy safety. This specific Clinical Pharmacist position is a member of the Pediatric Clinical Pharmacy Team at Monroe Carrell Jr. Children’s Hospital at Vanderbilt, primarily working with Neonatal ICU patients. Responsibilities include direct care through multidisciplinary interactions, optimizing pharmacotherapy, discharge planning, order verification, and responding to code and bedside procedures. Collaboration and excellent communication with pharmacy, nursing, and medical staff are essential. The role involves dayshift hours and a staffing requirement every 6th weekend.

Requirements

  • LIC-Pharmacist License - Tennessee
  • Relevant Work Experience Experience Level : 1 year
  • Bachelor's (Required)

Nice To Haves

  • Completion of ASHP accredited PGY-1 residency program, in a pediatric hospital setting or 2 years of pediatric hospital experience preferred.

Responsibilities

  • Provides leadership in a specialized healthcare team and the patient to assure optimal cost-effective drug therapy.
  • Developing specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient.
  • Leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy.
  • Works to continuously improve process and practice in the institution to enhance safety of medication therapy.
  • Provision of direct care through multidisciplinary interactions.
  • Optimizing pharmacotherapy.
  • Discharge planning.
  • Order verification.
  • Code and bedside procedure response.
  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
