About The Position

Work Schedule : 100% FTE. Day Shift. Hours ranging 7:00 AM – 5:00 PM. On-call rotation required. Hours may vary based on the operational needs of the department. Pay : Relocation assistance may be available for qualified applicants. Be part of something remarkable Join our team of respected experts in medication therapy management. We are seeking a Neuro-Oncology Clinic Pharmacist to: Manage comprehensive medication therapy for neuro-oncology patients, including treatment of comorbid conditions such as seizures, stroke risk, and cognitive decline. Collaborate with physicians under delegated authority to adjust therapy, manage adverse effects, and optimize treatment plans. Support clinical research initiatives by assisting with protocol-related medication management and ensuring safe and compliant drug use. Provide ongoing counseling and longitudinal follow-up to improve adherence to oral chemotherapy and supportive care regimens. Monitor, interpret, and manage laboratory values and treatment-related toxicities for complex chemotherapy regimens.

Requirements

  • Bachelor's Degree in Pharmacy or a PharmD degree
  • 1 year of clinical patient care and orders management experience
  • Licensed to practice pharmacy in the State of Wisconsin within 90 Days
  • Basic Life Support (BLS) within 90 Days

Nice To Haves

  • Completion of an ASHP Accredited Residency training program
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.)
