About The Position

Under the supervision of the Director of Pharmacy and clinical coordinator (if applicable), this role coordinates the activities of the pharmacy department to provide the highest quality of pharmaceutical care to all patients served by the hospital, while ensuring patient safety. The pharmacist will conduct 50% or more of their professional time in direct patient care activities, including interdisciplinary rounding, patient counseling, or participation in patient care committees. Provision of direct patient care encompasses activities such as the direct observation and evaluation of the patient and their medication-related needs; the initiation, modification, or discontinuation of patient-specific pharmacotherapy; and the ongoing pharmacotherapeutic monitoring and follow-up of patients in collaboration with other health professionals. The scope of patient care provided may include General Medicine/Surgical, Intensive Care Services, Obstetrics & Gynecology, Pediatrics, Emergency Care, Oncology, Peri-operative, Geriatrics and Outpatient Services.

Requirements

  • Current state Registered Pharmacist License.
  • B.S. degree in Pharmacy required.
  • Completion of postgraduate clinical training (PGY-1) or > 3 years of qualifying experience required.
  • BLS required.

Nice To Haves

  • Doctor of Pharmacy degree preferred.
  • Postgraduate clinical training PGY-2 preferred.
  • Board certification in a Board of Pharmacy Specialties clinical specialty preferred.
  • ACLS preferred.
  • Experience in hospital pharmacy operations preferred.
  • Oncology, infusion and/or sterile compounding experience.
  • Able to work independently from clinical and operations standpoint.
  • Comfortable talking to providers, making independent clinical decisions and troubleshooting day to day problems.
