About The Position

We are seeking an experienced Clinical Pharmacist – Inpatient Hospital for a 13-week contract in Zanesville, Ohio. The Pharmacist will provide inpatient pharmacy services while supporting safe and effective medication management across the hospital setting. Candidates must have hands-on experience with pharmacokinetic dosing, including vancomycin, and warfarin management. Experience with Epic Willow Inpatient is preferred but not required.

Requirements

• Minimum 1 Year of Recent Inpatient Hospital Pharmacy Experience Required.
• Current, Active Ohio Pharmacist License in Good Standing Required.
• Experience With Pharmacokinetic Dosing, Including Vancomycin, Required.
• Experience With Warfarin Management Required.

Nice To Haves

• Epic Willow Inpatient Experience Preferred.

Responsibilities

• Provide comprehensive inpatient pharmacy services and support safe, accurate, and appropriate medication therapy.
• Review medication orders and apply clinical pharmacy knowledge to optimize pharmacotherapy and patient safety.
• Perform pharmacokinetic dosing and monitoring, including vancomycin dosing and therapeutic management.
• Provide appropriate warfarin dosing and anticoagulation management based on clinical parameters and established protocols.
• Collaborate with physicians, nurses, and other healthcare professionals while maintaining accurate pharmacy documentation and supporting continuity of care.
