About The Position

We are seeking to expand our clinical pharmacy team with specialists in areas such as infectious diseases, internal medicine, critical care, pediatrics, and more. This role involves collaborating with interdisciplinary teams to ensure safe and effective medication use across all age groups (infant to geriatric).

Requirements

  • Graduation from an ACPE-accredited school of pharmacy with a Doctor of Pharmacy (PharmD) degree
  • Completion of ASHP-accredited PGY-1 Pharmacy Residency (required)
  • Board Certification by the Board of Pharmacy Specialties in primary area of practice (e.g., Pharmacotherapy (BCPS), Emergency Medicine (BCEMP), Infectious Diseases (BCIDP), Pediatric Pharmacy (BCPPS)) upon hire or within 12 months of hire (required)
  • Meet ASHP residency preceptor requirements (required)
  • Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certification upon hire or within 3 months of hire (required); Pediatric Advanced Life Support (PALS) if applicable
  • New York State Pharmacist Licensure and Registration (required)
  • Strong computer proficiency (MS Office, PowerPoint presentations)
  • Strong analytical and problem-solving skills
  • Leadership and interpersonal skills, including high emotional intelligence
  • Excellent English verbal/written communication skills

Nice To Haves

  • Completion of ASHP-accredited PGY-2 Residency in specialty area (preferred)
  • clinical pharmacy experience (at least 2-4 years) in a hospital setting and in relevant area of practice (required if no PGY-2 residency)
  • bilingual second language preferred.

Responsibilities

  • Leads assigned patient service lines, clinical areas and therapeutic programs.
  • Delivers direct patient care and clinical practice, including decentralized and service-based programs.
  • Using objective parameters and professional judgment, monitors and evaluates a patient’s medication regimen for efficacy, toxicity, side effects and outcome.
  • Conveys relevant findings and makes recommendation to other clinicians regarding a patient’s medication regimen utilizing both written and verbal communication.
  • Facilitates practice advancements by actively serves as a mentor and preceptor for Doctor of Pharmacy students and pharmacy residents.
  • Provides in-service lectures to medical, pharmacy, and/or nursing staff.
  • Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Performs focused research protocols and collaborates with other departments in research involving drug therapy.
  • Participates in hospital and departmental drug utilization evaluations and other quality assurance programs in addition to participating in hospital committees as required.
  • Detects, reports and participates in the management of adverse drug reactions and medical emergencies
  • Coordinates all aspects of medication therapy including nurse and physician roles in drug delivery and nursing techniques to ensure safe and effective delivery to the patient.
  • Documents all pertinent activities, interventions with other health care providers, recommendations, and the rationale for these recommendations, their impact, and/or outcome as related to patient care and/or pharmaco-economics.
