Requirements

  • Registered Pharmacist in the State of Florida
  • Pharm.D. from an ACPE accredited School of Pharmacy or B.S. Pharmacy Degree from an ACPE accredited School of Pharmacy WITH a required Board Certification (if no Pharm D.)
  • PGY-1 Residency Training or equivalent proven experience required (at least 3 years of pharmacy practice experience).
  • Experience in the provision of clinical pharmacy services, teaching and research.
  • Partially meets ASHP Residency Preceptor Requirements.
  • Obtains and maintains BLS-ACLS or PALS current certification as appropriate.

Nice To Haves

  • Board Certification preferred for a Pharm D. candidate
  • Board Certification required for a B.S Pharmacist Candidate.

Responsibilities

  • Interprets orders and verifies orders.
  • Accurately compounds and dispenses pharmaceuticals.
  • Monitors drug therapy regimens.
  • Provides clinical consultation and clarification to practitioners and suggests appropriate alternative therapy.
  • Supports the hospital formulary, rational drug therapy selection.
  • Attends and actively participates in daily multidisciplinary and bedside rounds.
  • Works with medical staff to promote rational drug therapy and make improvements identified by drug utilization evaluations.
  • Provides accurate and timely medication information.
  • Actively participates in pharmacy clinical services.
  • Participates in advanced antimicrobial management program.
  • Accurately and completely documents all clinical activities and interventions.
  • Supports and participates in the goals of the organization's quality improvement, risk management and patient safety programs to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
  • Participates in the training of new pharmacists, pharmacy students and technicians.
  • Maintains professional competency sufficient to meet current and future needs of the facility.
  • Aware of the patient population served and caters to their health care needs.
  • Provides basic technical support for automated dispensing machines and other pharmacy computer systems.
  • Effectively supervises pharmacy operation activities and provides support to the whole pharmacy team.
  • Compliant with facility pharmacy schedule to work physically at patient care areas to interact with medical staff, nurses and other members of the healthcare team.
  • Effectively and proactively follows-up with medical staff on any issues regarding drug therapy.
  • Trains and participates with Disaster Team drills and real-event response as assigned.
  • Trains and participates with Code Teams as assigned.
  • Obtains and maintains BLS-ACLS or PALS current certification as appropriate.
  • Practices and adheres to the “Code of Conduct” philosophy and “Mission and Value Statement”.
  • Maintains appropriate controlled substance records.
  • Works with the Pharmacy Director and appropriate manager to identify processes to meet or exceed all controlled substance diversion prevention and monitoring standards.
  • Complies with all regulatory standards.
  • Works with the Pharmacy Director and Operations/Clinical Manager to identify and develop processes to meet or exceed all regulatory standards.
  • Prioritizes work and leads others to comply with facility medication turn-around time policy and procedure.
  • Identifies areas for improved efficiency.
  • Participates in establishing pharmacy goals and objectives and assist in the implementation.
  • Participates with various nursing and medical staff committees as necessary.
  • Effectively supervises and assists pharmacy technicians and other support staff.
  • Acts as supervisor in charge as assigned.
  • Participates in the continuous quality review of order sets and/or computer drug entries.
  • Participates in the continuous quality improvement of pharmacy operations (operational/clinical).
