About The Position

The Clinical Pharmacist is an integral part of the multidisciplinary care team, functioning as a specialist in antimicrobial stewardship, medication safety, and optimized pharmaceutical dosing. This role involves assisting with medication histories, monitoring patient drug therapies, and providing drug information. The Clinical Pharmacist may also be called upon to respond to various codes (Code Blue/Sepsis/Stroke/STEMI), including the preparation of necessary medications. Additionally, they are responsible for filling medication orders, overseeing patient drug therapies, and supervising supportive staff. In the absence of the Director of Pharmacy, a clinical pharmacist may be required to assume the essential responsibilities and perform the duties of that role.

Requirements

  • Completion of a general 1-year pharmacy residency, specialty residency, and/or fellowship or at least 3 years of hospital pharmacy experience.
  • Ability to work independently and handle stress appropriately.
  • Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
  • Ability to set priorities, make critical decisions and respond quickly to emergency requests.
  • Ability to communicate effectively (verbal and written), preferably in both English and Spanish.
  • Knowledge of laws, rules and regulations that govern the practice of pharmacy.
  • Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics.
  • Must demonstrate commitment and adherence to STHS’s Compliance Program and Code of Conduct through compliance with all policies and procedures, the Code of Conduct, attendance at required training and immediately reporting suspected compliance issue(s) to the Compliance Officer.
  • BS or Pharm.D. from a School of Pharmacy.
  • Current license in good standing with Texas State Board of Pharmacy.
  • Must have and maintain BLS, ACLS, and/or PALS if applicable, or obtain within 6 months of hire.
  • ASHP IV certification or successful completion of an ACPE/ASHP accredited program within 180 days of hire if not already certified.
