About The Position

The Clinical Pharmacist I at City of Hope serves as the medication management expert, providing accurate drug information to providers, nurses, and other clinical staff. This individual will supervise technicians during their shift and assist with training and education for pharmacy staff. The role requires collaboration with colleagues and excellent customer service for patients and staff. Additional duties may include quality and performance improvement activities as determined by Pharmacy Leadership. City of Hope is a national system dedicated to fighting cancer, diabetes, and other life-threatening illnesses, with locations across Southern California, Atlanta, Chicago, and Phoenix.

Requirements

Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending
Current CA Pharmacist License
BLS certification through the American Heart Association

Responsibilities

Collaborates with providers, nurses, and other clinical staff to ensure the medication management process is safe, high quality, and cost-effective for all patients.
Provide oversight of safe and timely medication use system.
Oversee compounding in alignment with USP, Board of Pharmacy, and other regulations.
Ensure medications are received, stored, labelled, transported, and wasted appropriately.
Ensure controlled substance processes are adhered to.
Collaborate with members of multidisciplinary health care team to optimize patient’s medication therapy.
Participates in care coordination and Investigational Drug management for research patients, following all required regulations and practices.
Actively supports reporting of Pharmacy Interventions, Adverse Drug Reactions, and Medication Errors.
