About The Position

As our Clinical Pharmacist, you’ll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury. Every day, you’ll build trust in these interactions by reviewing every prescription in your queue. We never rely entirely on the computer to check for safety; we always use an expert for the final check. That’s you. You’ll review prescriptions for potential drug interactions, correct dosing, patient allergies or anything that looks unusual in your opinion. To excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for our patients. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.

Requirements

  • Bachelors Of Science Pharmacy and 1 year of pharmacist experience, upon hire or Doctor Of Pharmacy Pharmacy 1 year of pharmacist experience
  • Pharmacist: AZ, upon hire
  • Basic Life Support - CPR, upon hire
  • Board Certified Ambulatory, upon hire or Board Certified Cardiology, upon hire or Board Certified Compound Strl, upon hire or Board Certified Critical Care, upon hire or Board Certified Emergency, upon hire or Board Certified Geriatric, upon hire or Board Certified Infectious Dis, upon hire or Board Certified Nuclear, upon hire or Board Certified Nutrition Supp, upon hire or Board Certified Oncology, upon hire or Board Certified Psychiatric, upon hire or Board Certified Pediatric, upon hire or Board Certified Pharmacotherapy, upon hire or Board Certified Organ Trans, upon hire

Responsibilities

  • Demonstrates ability to document clinical activities and interventions such as consults, patient teachings and other patient specific information as defined by policy and procedure.
  • Interprets medication orders and/or prescriptions for drug interactions, appropriate drug selection and appropriateness. Accurately enters / verifies the order or prescription into the computer system, labels, and dispenses medications in a timely manner.
  • Verifies and checks medications prepared by other pharmacy personnel.
  • Communicates drug information to patients and health professionals. Resolves drug related problems by consulting with other health care professionals to achieve appropriate patient outcomes. Unclear orders written by a licensed provider are clarified with the licensed provider. Assists the licensed provider in appropriate drug selection and therapeutic regimens.
  • Achieves and maintains an adequate basic knowledge base to provide clinical services for all age groups served. Provides basic clinical services as required by policy and procedure.
  • Maintains a working knowledge of automated systems
