About The Position

This is an entry-level clinical pharmacist position within a High Reliability Organization (HRO). The clinical pharmacist will support the vision, mission, and goals of YNHHS and the Department of Pharmacy, demonstrating competency in all basic clinical pharmacist skills. The role requires adherence to departmental policies, procedures, protocols, and guidelines, and involves medication order review, competency in pharmacy automation, and contributing to cost-reduction/revenue generation. Additionally, the position involves providing education and training to other pharmacy staff, students, residents, and healthcare team members.

Requirements

  • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree
  • Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut
  • Demonstrate basic knowledge of pharmacology and therapeutics
  • Understand pharmacy systems of drug distribution and formulary management
  • Excellent verbal and written communication skills
  • Ability to work with others

Nice To Haves

  • Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience
  • Hospital pharmacy experience

Responsibilities

  • Demonstrate and promote CHAMP behaviors
  • Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signs
  • Demonstrate competency in pharmacy automation and technology as applicable to work area
  • Adhere to all departmental policies, procedures, protocols, and guidelines
  • Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team members
  • Contribute to departmental cost-reduction/revenue generation
