About The Position

This role focuses on coordinating clinical outcomes for nursing, emphasizing a collaborative approach with physicians and allied health providers. The coordinator will be involved in patient care planning, discharge facilitation, and process improvement initiatives. The position is based at Texas Health Fort Worth, an 851-bed, Magnet-designated hospital serving the Fort Worth community since 1930, offering advanced care in various specialties and recognized for its high-acuity capabilities and commitment to training future physicians. The hospital is Joint Commission-certified as a Comprehensive Cardiac Center, a Comprehensive Stroke Center, and a Level I Trauma Center, and is recognized as the Best Place to Have a Baby in Tarrant County.

Requirements

  • Master’s Degree Nursing required
  • 2 years’ direct patient care experience Required
  • 5 years of current hospital experience Required
  • Current RN licensure or compact licensure recognized by the Texas Board of Nursing upon hire
  • BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly Required
  • CPI - Crisis Prevention Intervention Training Maintained Annually 90 Days Required
  • Other CNL or other professional nursing certification within 12 Months of hire required

Responsibilities

  • Collaborates with physicians and/or APRN for services needed as well as all with other allied health care providers
  • Partners with physicians by engaging in rounding to discuss priority of patients and patient care needs
  • Participates in interprofessional rounds, leading when appropriate
  • Collaborates with care transitions team to facilitate discharges, RCB measures, review of excess days and patient throughput
  • Facilitates the development, implementation, monitoring, and assessment of processes that impact patient outcomes
  • Uses benchmark data from internal and external sources to identify opportunities for improvement of care and patient safety
  • Incorporates the quality caring model in daily nursing decisions
  • Participates in shared decision making by participating in councils as requested by nursing leadership
