About The Position

As a Clinical Navigator at PAM Health, you will be the face of our hospital, meeting people during challenging times and guiding them towards recovery. You will share our success stories and the potential for success during their rehabilitation stay, empowering them to achieve their goals with the support they need. This role is crucial in helping patients understand their potential and the support available to them during their rehab journey.

Requirements

Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required
Must have a minimum of two years’ experience in the designated field of license
preferably with rehabilitation/LTACH and marketing experience

Nice To Haves

Degree in a health-related field from an accredited college or university preferred
Prior marketing and/or rehabilitation/LTACH experience preferred

Responsibilities

Cultivates referral relationships
Manages assigned territory
Completes patient assessments
Makes sales contacts
Analysis of referral and admission data from assigned territory
Works collaboratively with business development team members including the Admissions Manager and admission staff, as well as nursing and other internal and external staff to facilitate the referral conversion
Provides feedback and recommendations for program development and quality improvement initiatives related to customer service, the referral and admission process and patient and referral source satisfaction
Develops and maintains excellent relationships to all stakeholders including prospective patient, family members, physicians, ICU and floor nurses, discharge planners, case managers and payer representatives
