About The Position

The Clinical Manager is responsible for the day-to-day triage of team activities, including accepting referrals, ensuring timely admissions, assigning patients to appropriate team members, and monitoring patient care according to the interdisciplinary plan of care. This role involves conducting Interdisciplinary Team (IDT) meetings, supervising and supporting clinical staff through evaluations and field visits, and ensuring the ongoing development of the team's clinical skills. The Clinical Manager is accountable for maintaining quality care and compliance with state and federal regulations, CHAP accreditation standards, and organizational policies and procedures. This position supervises Case Managers, RNs, LPNs, CNA/HHAs, Therapists, Social Workers, Grief and Bereavement Coordinators, and Chaplains.

Requirements

  • At least one year of Hospice/Home Health experience.
  • At least a RN from a NLN accredited nursing program.
  • Licensure from the governing body for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia.
  • CPR certification is required.

Nice To Haves

  • A minimum of two years of management experience.
  • At least four years of medical survival and/or oncology experience.
  • A BSN is preferred.

Responsibilities

  • Receives/triages inquiries from the Referral Center, staff, patients, caregivers, referral sources, and the community.
  • Performs telephone assessments to determine caller needs and develops/implements a plan to address them, which may include instruction, dispatching staff for patient visits, or contacting a physician.
  • Ensures that interdisciplinary team members possess the necessary skills and competencies to care for assigned patients.
  • Acts as a liaison between Hospice and other professionals, including physicians, hospitals, nursing homes, and agencies, assisting the Executive Director.
  • Enhances the integrity and clinical practice of the team by serving as a role model, mentor, instructor, and resource person.
  • Takes responsibility for personal professional growth and development to maintain and improve competence, evaluating personal needs for support and utilizing identified systems to meet them.
  • Develops new skills through ongoing education and staying current with nursing practice via journals and literature review.
  • Participates in quality improvement activities, the QUAPI program, and hospice-sponsored inservice training.
  • Serves on committees or teams as approved by the Executive Director.
  • Ensures compliance with all federal, state, and local laws and regulations, CHAP accreditation standards, Conditions of Participation, and organizational standards of care, policies, and procedures.
  • Communicates with physicians regarding patient condition, incorporating hospice expertise.
  • Implements physician's orders by documenting and communicating them to staff, patients/caregivers, or facilities.
  • Utilizes appropriate resource services (e.g., ambulance, IV therapy, DME, inpatient facilities, agencies) to implement orders.
  • Performs introductory (90-day) and annual performance reviews of assigned team members according to organizational policy and timelines.
  • Ensures licensed practical nurses, nursing assistants, and other care assistants work under the direct supervision of a registered nurse.
  • Follows all rules and regulations of the facility.
  • Assigns and coordinates daily schedules of clinical staff, including PRN staff.
  • Facilitates the provision of patient care services on a day-to-day basis according to the plan of care.
  • Works closely with team members to meet agency productivity standards.
  • Coordinates all interdisciplinary team services, including on-call personnel, with the after-hours department to ensure 24-hour availability of patient care services.
  • Assists the Executive Director in assessing the need for new staff and in recruiting, interviewing, and making hiring decisions in collaboration with Human Resources.
  • Orients new staff to the team after HR and clinical orientations.
  • Coordinates with Clinical Education staff (CORE) to ensure assigned staff attend and receive education programs.
  • Coordinates with staff in contract bed facilities regarding the hospice plan of care, standing orders, etc.
  • Assists with the implementation of a plan to address identified needs.
  • Collaborates and communicates with facility staff and CNAs regarding patient status and care provided.

Benefits

  • Pay Range: $100,471.88 - $149,542.35
