The Clinical Manager is responsible for the day-to-day triage of team activities, including accepting referrals, ensuring timely admissions, assigning patients to appropriate team members, and monitoring patient care according to the interdisciplinary plan of care. This role involves conducting Interdisciplinary Team (IDT) meetings, supervising and supporting clinical staff through evaluations and field visits, and ensuring the ongoing development of the team's clinical skills. The Clinical Manager is also responsible for ensuring quality care and compliance with state and federal regulations, CHAP accreditation standards, and organizational policies and procedures. The position supervises Case Managers, RNs, LPNs, CNA/HHAs, Therapists, Social Workers, Grief and Bereavement Coordinators, and Chaplains.
