This position reports to the Regional Head of Clinical Field Operations, the Nurse Practitioner Lead is responsible for recruitment, training and management of Registered Nurses (RNs), Nurse Practitioners (NPs), Licensed Clinical Social Workers (LCSWs), and Physician Assistants (PAs), ensuring effective collaboration, optimal staffing, and high-quality patient care delivery to Care at Home family members transitioning to home health care. This role involves strategic planning, team leadership, performance monitoring, in partnership with the Community Care Manager, overseeing an interdisciplinary team to meet organizational goals, improve member outcomes, and provide exceptional patient care. The Nurse Practitioner Lead will also develop processes and policies to ensure members, and their families, receive timely and effective navigation support, both telephonically and in the field.
