About The Position

Select Specialty Hospital - Kansas City is a Critical Illness Recovery Hospital (LTACH) seeking a per-diem Clinical Liaison (RN or RT). This role involves advocating for referred patients by collaborating with clinical teams to assess needs, serving as a resource for healthcare professionals by fostering relationships within medical facilities, coordinating care plans with physicians and families, facilitating patient transfers, and addressing barriers to accessing specialized post-ICU care. Requirements

  • Registered Nurse (RN) or Respiratory Therapist (RT) License.
  • Ability to complete clinical assessments of medically complex patients.
  • Demonstrated computer proficiency.
  • Must exercise sound judgment in handling the professional/confidential nature of health care sales.

Nice To Haves

  • Will consider other designations like LPN, PT, OT, etc., upon further review.
  • Two years of related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role are preferred.
  • Demonstrated competency with the preadmission process, including assessments and relationship building in a rehabilitation setting or in another post-acute care setting, is preferred.
  • Proven track record in sales and marketing with a least one year of experience in a related position preferred.

Responsibilities

  • Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing strong clinical background, payer knowledge, and problem-solving skills.
  • Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
  • Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective level of care.
  • Serve as a resource for healthcare professionals through fostering relationships within medical facilities.
  • Coordinate care plans with physicians and families.
  • Facilitate patient transfers.
  • Address barriers to accessing specialized post-ICU care.
