About The Position
Now hiring for a Clinical Liaison (RN/LPN/Therapist) in Lake Charles, LA! Join an exceptional team to navigate the full post-acute care continuum! The Clinical Liaison will play an integral role in our team by ensuring care continuity of patients and promoting our care continuum.
Requirements
- Clinical discipline with current license in good standing
- 2 or more years of clinical experience required
- Working knowledge of computers and software packages such as Microsoft Office
- Knowledge of hospital data bases
- Proven ability to work with physicians, social workers, insurance companies, case managers, patients, and families
- Current valid LA Driver’s License
Nice To Haves
- Management experience preferred
- Post-Acute care experience preferred
Responsibilities
- Ensuring care continuity of patients
- Promoting our care continuum
- Supervising and participating in Marketing and Admissions Intake activity with minimal direction
- Designing and achieving appropriate Marketing/Admissions Intake goals and objectives
- Networking with referral sources
- Developing Marketing plan for hospital
